About Direction Direction™ is unique to Gray Healthcare, a highly individualised model promoting recovery with complex individuals. In a complex world of revolving door syndrome and confusion it provides support and Direction in ones life. It is tailored towards the needs of each individual service user, and uses a bespoke risk management package that address early warning signs and ensures service users receive the appropriate treatment at the appropriate time. Using the Recovery Star and the areas identified - service users are able to plot and plan their progress through their chosen care pathway.
This is achieved by a highly experienced clinical mental health team working effectively with the service user, their support staff and their Community Mental Health Team (CMHT). Each service user receives at least 4 hrs of 1-1 dedicated support worker time per day, helping them through their recovery programme. A Registered Mental Nurse (RMN) for one clinical session per week for mental state assessment using the Brief Psychiatric Rating Scale (BPRS), an OT one clinical session per 2 weeks, using the Model of Human Occupation (MOHO) assessment process and developing an individualised therapy programme in conjunction with the service user to enable them to achieve their goals. Finally psychological support is provided by Cognitive Behavioural Therapy (CBT) sessions of one per two weeks, to address any psychological issues.
Our staff are trained in recognising individual relapse signatures and report with a daily risk assessment to the registered manager who, if required, will ensure a visit from a registered mental nurse that morning. Linked with an out of hours on call service an experienced RMN is always available to ensure risk is managed effectively. Short notice need for enhanced staffing is available 24hrs per day.
