What are the Expected Outcomes? - Service User: For the Service User,Touchdown™ is a step-by-step plan designed for their individual needs. SHINE™ is a structured programme of proven medical and therapeutic inputs which aims to build or re-build the daily living skills, insight, education and confidence required to safely re-integrate back into the community.
- NHS Care Co-ordinator: For the NHS Care Co-ordinator, Touchdown™ provides a measurable programme of therapy inputs that can be used to evaluate a Service Users progress as well as challenges. Regular CPAs and Touchdown™ progress reports ensure that the NHS Care Co-ordinator is at all times well informed about their Service Users level of care and progress, and jointly developing new working strategies.
- Primary Care Trust: For the PCT, SHINE™, with Touchdown™ are transparent, multi-disciplinary models that provide an audit of inputs and results at each stage of a Service Users Care Plan – from the initial assessment to the successful discharge of the patients from our services. Gray Healthcare also helps a PCT to lower costs by moving appropriate Patients, who no longer require costly secure environments, into intensive, home based rehabilitation along the next stage of their Care Pathway.
- Progress Milestones: Each stage of a Service Users SHINE™ Plan has a number of recovery goals and milestones. Examples include success in 10 measured Touchdown™ daily living skills categories which include education, social involvement, domestic skills and budgeting as well as a self-medication programme and ultimately discharge from specialist services etc. In this way, a Service Users progress and preparation for the next step of recovery and independent living as part of their Care Pathway is clearly identified.
Finally, we recognise that a safe return to the community can be a major step for the Service User. Gray Healthcare works closely with teams of Social Workers, Care Co-ordinators and the PCT to ensure that the Patient has the appropriate support network and Direction™ to help them to manager their lives as independently as possible. Our goal is a safe and sustainable return to living in the community and to provide an alternative to the “Revolving Door” syndrome.
