Key Indicators and Review Triggers (These are examples of the review mechanism only)

1. I don’t look after myself
a. Not looking after yourself at all and not engaging in self care and hygiene
b. Without prompting, support and assistance sleeping, eating and exercise will be poor and self care and hygiene inappropriate or not happening
c. May be self neglecting to a point of self harm
d. Does not engage in discussion about self care andmay refuse help
f. Often declines intervention or treatment for physical conditions

2. I don’t look after myself but sometimes realise that I could and this makes me feel bad
a. As in 1 above but occasionally aware that you don’t like how things are
b. Feeling does not last long enough for staff to engage and help
c. Change of mind before self cares can be implemented
d. Sometimes change of mind may be accompanied with verbal outburst

3. I remember not feeling this bad – now I want help
a. Is prepared to discuss well being and self cares and accept limited help with physical cares
b. Recognises there is a need to change self care and hygiene process
c. Recognises that this change cannot be effected without support

4. I have a support package to help me feel better
a. Is taking positive action on identified key areas but the initiative has to be taken by staff with
prompting and support
b. Still quite dependent on others to create and maintain healthy living and self care patterns
c. No inner sense of how to create sense of well being through effective self care
d. Would not readily make the first move towards self care and hygiene

5. I want to take responsibility for looking after myself better
a. Has actively engaged in self care
b. Is able to discuss and explore what gives a feeling of well being and what adversely affects this
c. Is able to understand how sleep diet and exercise can positively and negatively affect self care,
hygiene and well being
d. Is beginning to address personal hygiene, presentation and grooming if they were issues in the past
e. Has stated a commitment and resolve to make positive changes
