Review Mechanisms

Each Service Users Care Plan and treatment programme is subject to a strict review process.

We believe implicitly in measured results and a transparent partnership with the Service User, their family, Carers, and the Primary Care Trust. We, therefore, have robust Review Mechanisms that both audit our Care Plans as well as provide frequent opportunities for all stakeholders to amend the course of treatment.

Internal Review Mechanisms

  • Daily Risk Assessment
  • Weekly Progress Review By A Named Nurse
  • Person Centred Care Plans
  • 2 Weekly Progress Report by Occupational Therapist
  • 2 Weekly Progress Report by Psychological Team
  • 4-weekly Touchdown Review Assessment to monitor Patient safety and progress

External Review Mechanisms

  • CPAs – minimum 4 times per year
  • 117 Meetings
  • Advocacy Services – Provided through “Advocacy Options” and includes face to face meetings and monthly telephone contact
  • Independent Specialists – Solicitors, Social Workers, Religious Advisors
